Registration
New Students Will Need the Following for Registration
- The child's social security number or card;
- A birth certificate, or other proof of age;
- An up to date record of immunizations as required by Colorado law;
- Addresses and phone number(s) of the child's doctor and dentist as well as of relative or friends who can be contacted in case of emergency.
- A completed registration form. This can be downloaded for new students or for returning students and filled out in advance, or we will provide one when you arrive in person.
Required Immunizations
Immunization requirements can vary from year to year. Current requirements can be found on the Colorado Department of Health website.
New students who do not comply have 14 days to comply or will be sent home from school. An exemption form is available for medical, religious, or personal reason.
