Student Registration
New Students Will Need the Following for Registration
- The child's social security number or card;
- A birth certificate, or other proof of age;
- An up to date record of immunizations as required by Colorado law;
- Addresses and phone number(s) of the child's doctor and dentist as well as of relative or friends who can be contacted in case of emergency;
- A completed set of registration documents.
You can download the complete registration package here. None of this needs to be filled out before you come into the school, but you may save time by filling out some or all of the forms in advance. in any case, please look them over in case you have any questions.
Required Immunizations
Immunization requirements can vary from year to year. Current requirements can be found on the Colorado Department of Health website.
New students who do not comply have 14 days to comply or will be sent home from school. An exemption form is available for medical, religious, or personal reasons.
